The Licensed Practical Nurse (LPN) is responsible for facilitating clinic-based access to addiction medications. The LPN is responsible for scheduling of initial and follow-up appointments for medication management for assigned patient caseload. The Licensed Practical Nurse provides care coordination and addresses barriers to effective care for patients receiving addiction medications.

The Licensed Practical Nurse (LPN) is responsible for facilitating clinic-based access to addiction medications. The LPN is responsible for scheduling of initial and follow-up appointments for medication management for assigned patient caseload. The Licensed Practical Nurse provides care coordination and addresses barriers to effective care for patients receiving addiction medications.
Specific Duties & Responsibilities
- Conducts screening for substance use disorders, mental health disorders and infectious diseases.
- Assesses and records vital signs as requested by prescriber.
- Assess clients for possible signs of intoxication prior to dispensing medications.
- Provides patient education about addiction medications.
- Conducts medication administration of partial agonist medications during induction and stabilization under
- Administers long-acting injectable medications in accordance with prescriber’s orders.
- Document in Medication Administration Record (MAR) and clinical record in accordance with agency policy and applicable regulatory standards.
- Conducts spot checks of individual medication supply to assure adherence.
- Conducts venipuncture to obtain blood specimens, obtains urine or saliva samples for toxicology testing.
- Obtains daily test results from clinical laboratory and assure that all lab reports are filed in the patient record.
- Alerts prescriber and clinical team immediately upon identification of medication adherence concerns.
- Alerts prescriber immediately upon receipt in the event of abnormal lab values.
- Fax, phone, electronically deliver, or by any other practical means, advise pharmacy of medications to be filled.
- Receives, checks and secures medication deliveries.
- Responsible for conducting medication inventory at start and end of each shift.
- Attend regularly scheduled staff meetings and case conferences.
- Attend all required in-service training seminars.
- Participate in quality improvement activities.
- Other duties as required.
- Minimum of (1) year experience working in a behavioral healthcare service setting preferred.
- Current New York State LPN License required.
- Proficiency with computer operation (Microsoft Word, Excel, AWARDS, and Outlook programs).
- Must be able to work a flexible schedule including weekends and evening when needed.
