Position Description

Job Title: Registered Nurse (Maternal Health Program) (Bilingual in Spanish)

Location: Staten Island, New York (Field Visits Required)

Overview:

We are seeking a Registered Nurse to join a maternal health program, which supports high-risk, low-income first-time mothers and their families. The Registered Nurse will work closely with pregnant women and infants, addressing medical and social needs while following personalized care plans. This position involves delivering essential education, case management, and home visits to families from pregnancy through the child’s second birthday.

The Registered Nurse will report directly to the Nurse Supervisor and will play a key role in ensuring that families receive the comprehensive care they need. This is a unique opportunity to make a tangible difference in the lives of vulnerable families.

Salary: $90,000 annually

Key Responsibilities:

Home Visitation & Case Management:

  • Conduct home visits to pregnant and parenting women in the Staten Island area.
  • Provide case management, education, and resources to help families from pregnancy through the child’s second birthday.
  • Manage a caseload of approximately 25 families, balancing home visits with team collaboration and support.

Clinical Duties:

  • Build rapport and therapeutic relationships using motivational interviewing and active listening techniques.
  • Assess the physical, emotional, social, and environmental needs of clients, aligning care with established guidelines.
  • Set client-specific goals and assist families in achieving them through education, referrals, and resource connections.
  • Collaborate with other healthcare professionals to ensure comprehensive care and follow-up.
  • Maintain accurate documentation for Medicaid billing and required reports.

Qualifications:

Required:

  • Bachelor’s Degree in Nursing (BSN) or currently enrolled in a BSN program.
  • Valid New York State RN license in good standing.
  • CPR certification.
  • Bilingual in English and Spanish.
  • Minimum of two years’ experience in maternal/child health, public health, or home visiting is preferred.
  • Strong written and verbal communication skills.
  • Basic computer proficiency.
  • Must have a valid driver’s license and access to an insured vehicle.
  • Flexibility to work evenings and weekends as needed.

Preferred:

  • Experience in maternity, labor, or delivery nursing.
  • Previous experience in home visiting or fieldwork.

Work Schedule:

  • Full-time (35 hours/week)
  • Monday to Friday, 9:00 AM to 5:00 PM, with some flexibility
  • Around 6 months of training, including live virtual sessions and in-person shadow visits

  • Compassionate, empathetic, and skilled at building strong, trusting relationships with clients.
  • Eager to be trained in the home visitation model and apply it effectively.
  • Strong time management skills and the ability to manage a caseload efficiently.
  • Able to work independently while also collaborating as part of a supportive team.

Additional Information: This position combines in-person home visits with telehealth consultations, promoting a relationship-based care model. New nurses will receive comprehensive training, including shadowing experienced team members and engaging in virtual learning sessions. Candidates should be comfortable with technology and thrive in a dynamic, fast-paced environment.
